I think the duct tape wouldn't do anything if I have a ball gag in. Plus the ball gag would push the panties back and almost choke me.

If the panties were wrapped securely around the ball, they wouldn't be pushed anywhere. They would remain wherever the ball gag is. I was thinking though, with the tape over the front, it would be more difficult to make sounds, but I suppose a safer option could be to tape the panties to the ball gag so all three could be used, without the tape being placed over the front of the ball gag.
