I can think of a few.

1. They make sure that the actress is right for the scene. So if you see a woman sitting down on a 5 inch plug, it's safe to say that this is somewhat normal for her, cause it wouldn't be the same with someone who was not used to it.

2. They don't film all the preparation in getting someone's ass pliable and stretched enough to fit what's going in. Even for a pro, there are preliminaries... otherwise they'd have to wear a diaper all the time.

3. If you see someone spit on another's ass and then put something huge in it... they're not showing you reality. Asses don't lubricate on their own and spit won't cover enough or last enough.

In my experience I've taken a 2inch wide, 7 inch long dildo with little problem. Didn't use much lube and there wasn't any working up to it. So I guess it depends on the person as well.
